Including patient preferences and expectations in shared decision-making may improve treatment satisfaction, adherence, and persistence.Purpose: To investigate long-term alignment of individual preferences and expectations of pwMS with their actual DMT and its effect on treatment satisfaction, health-related quality of life (HRQoL), adherence, and treatment discontinuation.Methods: A total of 401 pwMS beginning a new DMT were enrolled from 2015 to 2018 in a non-interventional study at three German MS centres. Patient preferences regarding DMT, TSQM-9, SF36, and self-reported adherence as well as relapses and EDSS were recorded at baseline and every 3 to 6 months for up to 3 years.Results: Efficacy and tolerability were the highest-ranking preferences at baseline. Actual selection of DMT corresponded more closely to safety than efficacy, tolerability, or convenience preferences. Participants reported excellent adherence throughout the study. DMT persistence was 69.0%, with earlier discontinuation for injectable vs oral or infusion therapies. Breakthrough disease, rather than patient-reported outcomes, was the main driver of DMT discontinuation. For all routes of administration, global treatment satisfaction increased over time despite lower satisfaction with convenience. Several patterns of changing preferences were observed.Conclusion: This study provides insight into the interaction of DMT preferences of pwMS with their actual treatment experience. 38. Giant Tumors of the Breast in Adolescence: Clinical Algorithm for Reconstruction of Young Patients in Thelarche

Abstract

Introduction: Breast tumors such as cystosarcoma phylloides and fibroadenoma may show rapid growth and occur early in thelarche. In adolescents, tumor growth often remains unnoticed due to a lack of breast awareness or a sense of shame in these young girls. The necessity for surgical intervention may interfere with the normal development of the breast, however in certain cases has to be performed due to the threat of deformation of the growing breast or the malignant potential of some tumors like cystosarcoma phylloides. However, visible scars have to be avoided at any price in these young girls, as they are still in their pre-marital and psychologically vulnerable phase. The presumed size of the breast after completion of all Tanner stages of breast development has to be taken into consideration while planning the immediate or deferred reconstruction. Methods: We report on three young girls, 13, 14 and 16 years of age, who presented with a giant cystosarcoma phylloides of 9,6 cm (190 grams), a giant fibroadenoma of 14 cm (287 grams) and a giant PASH-tumor of 16,3 cm (957 grams). All patients were in their thelarche. The extension of the tumor was almost to the whole circumference of the breast with close borders to the skin envelope, resulting in a marked anisomastia due to the giant tumor size. We are proposing here scar-sparing procedures adapted to the form of the breast and the BMI of the patients. For access to the tumor, in two cases, we chose the anterior axillary line to avoid visible scars on the surface of the breast - one with autologous and one with heterologous, implant-based reconstruction. In the third case, the patient had a marked anisomastia with a massively ptotic breast with a huge broadening of the nipple-areolar-complex due to the growth of the tumor. Here we selected a tumor-size adapted, inferior-pedicled reduction mammaplasty according to Ribeiro in the modification of Rezai with a skin reduction and shortening of the diameter of the areola. Patient reported-outcome has been measured by validated Breast-Q-questionnaires. Results: We present three different types of access for the removal of giant tumors and three options for the reconstruction of patients in their thelarche. Two tumors were removed through axillary access to the breast to avoid scars in the developing breast. We achieved free margins at first surgical intervention in all three cases. One reconstruction was performed by autologous tissue through lateral thoracic wall advancement mammaplasty. Another patient had an implant-based reconstruction. The third patient – who presented with a macromastia with marked enlargement of the nipple-areola-complex was treated with a tumor-adapted reduction mammaplasty. All three young patients have reported a high degree of satisfaction in validated Breast QoL questionnaires with the outcome of the procedures.

Abstract

INTRODUCTION: Chemotherapy-induced polyneuropathy (CIPN) and post-chemotherapy cognitive impairment (PCCI) are frequent side effects of paclitaxel treatment. CIPN/PCCI are potentially irreversible, reduce quality of life and often lead to treatment limitations, which affect patients’ outcome. We previously demonstrated that paclitaxel enhances an interaction of the Neuronal calcium sensor-1 protein (NCS-1) with the Inositol-1,4,5-trisphosphate receptor (InsP3R), which disrupts calcium homeostasis and triggers neuronal cell death via the calcium-dependent protease calpain in dorsal root ganglia neurons and neuronal precursor cells. Prophylactic treatment of rodents with lithium inhibits the NCS1-InsP3R interaction and ameliorates paclitaxel-induced polyneuropathy and cognitive impairment, which is in part supported by limited retrospective clinical data in patients treated with lithium carbonate at the time of chemotherapy. Currently no data are available from a prospective clinical trial to demonstrate its efficacy. METHODS AND ANALYSIS: The PREPARE study will be conducted as a multicenter, randomized, double-blind, placebo-controlled phase-2 trial with parallel group design. N = 84 patients with breast cancer will be randomized 1:1 to either lithium carbonate treatment (targeted serum concentration 0.5–0.8 mmol/l) or placebo with sham dose adjustments as add-on to (nab-) paclitaxel. The primary endpoint is the validated Total Neuropathy Score reduced (TNSr) at 2 weeks after the last (nab-) paclitaxel infusion. The aim is to show that the lithium carbonate group is superior to the placebo group, meaning that the mean TNSr after (nab-) paclitaxel is lower in the lithium carbonate group than in the placebo group. Secondary endpoints include: (1) severity of CIPN, (2) amount and dose of pain medication, (3) cumulative dose of (nab-) paclitaxel, (4) patient-reported symptoms of CIPN, quality of life and symptoms of anxiety and depression, (5) severity of cognitive impairment, (6) hippocampal volume and changes in structural/functional connectivity and (7) serum Neurofilament light chain protein concentrations. ETHICS AND DISSEMINATION: The study protocol was approved by the Berlin ethics committee (reference: 21/232 – IV E 10) and the respective federal agency (Bundesinstitut für Arzneimittel und Medizinprodukte, reference: 61-3910-4044771). The results of the study will be published in peer-reviewed medical journals as well as presented at relevant (inter)national conferences.
